Financial Hardship Reasons and Proper Documentation

To be eligible, you must be able to provide evidence that you are experiencing a financial hardship. In order to be considered for funds, this documentation must be verifiable. Being verifiable means our support team must be able to determine that your information is true, valid and correct. 

Acceptable documentation includes:

  • A letter from your employer on company letterhead providing written approval of a leave of absence, furlough or otherwise, whether it is paid or unpaid, and effective dates.
  • Some campaigns require you to meet a certain income guideline. In these cases, you may need to provide income verification that shows you are within the allowable income limit. 
  • An email from your employer with a company-provided email address showing written approval of a leave of absence, furlough or otherwise, whether it is paid or unpaid, and effective dates.
  • Shift cancellation notices from your employer, including effective date and end date (or notification that the cancellation is indefinite).
  • A side-by-side picture of your last two pay stubs (dated this year) showing a decrease in hour and/or pay. 
  • A letter from a medical professional excusing you from work, providing hospital information, such as the name of the physician, and hospital name/address/phone number.
    • Important Note: Medical release information including conditions and personal medical history is not necessary, and not requested, per HIPPA policy.
  • Handwritten letters are not accepted. 
  • Other forms of evidence accepted so long it includes verifiable information. 

All of these documents must also clearly show your name and/or other personally identifiable information, such as address. 

How can I get verifiable documents?

Financial Hardship looks different to everyone, which is why we are flexible with what we accept. However, submitted evidence must include a way for our support team confirm the information if and when we need to. The most common way we can verify this information is when submitted evidence includes one or more from the following:

  • Employer/Business details: Name, address, phone, email
  • School/Daycare details: Name, address, phone, email
  • Trade Industry: Applicant tradesman license (hair salon, barbershop, taxi, construction, etc..)

All evidence must fall within one or more of the financial hardship examples below. 

What are some examples of a financial hardship, and what to provide?

  • Unpaid leave due to mandatory isolation
    • Documentation from your employer confirming the reason for leave of absence, and whether it is paid or unpaid.
  • Unpaid leave due to voluntary isolation in order to provide care for vulnerable or infected relatives and/or dependents
    • Documentation from your employer confirming the reason for leave of absence, and whether it is paid or unpaid.
  • Loss of wages due to involuntary cancellation of work or reduction of hours
    • A layoff or shift cancellation notice from your employer. Notice(s) must include an effective date and end date or that the time period is indefinite.
  • Unpaid leave of absence due to school or daycare closure
    • Documentation from your child’s school/school district verifying closure and a notice from your employer confirming the reason for leave of absence, as well as if it is paid or unpaid.

Examples of Unacceptable Documents

While we understand you want to be able to show proof that you are experiencing a hardship, certain documents/photos cannot be accepted because they can’t be verified properly.

Some examples include, but are not limited to:

  • Handwritten letters from employers
  • Photographs of any kind depicting hardship (e.g., photos from a hospital, workplace closures, photos of self etc.) other than those listed above as acceptable
  • Text message/social media conversation screenshots
  • Any kind of documentation that does not provide details that our support team could easily verify (e.g., no employer name, address, phone number, any kind of screenshots showing a blank schedule or otherwise etc.).
