Am I eligible to apply?

To be eligible for the Suburban Cook County Family Support Fund fund, you must meet all of the following requirements and provide evidence of such:

  • Reside in suburban Cook County, IL
  • Have a household income at or below 250% of the Federal Poverty Level 
  • Have experienced a COVID-19 hardship, including:
    • Lost job/had hours and/or income reduced due to COVID-19
    • Have taken an unpaid leave to:
      • Care for a vulnerable or infected relative
      • School or daycare closures 
      • Having to wait for COVID test result
      • COVID illness

You will need the following documents to apply:

 

  • An unexpired proof if identification, such as:
    • Driver's license
    • Government-issued identification card
    • Passport
  • One form of government issued identification that verifies your current residential address or two alternative forms of identification
  • Proof of your COVID-19 hardship due to unpaid leave, care for vulnerable or infected relatives, or loss of wages due to business or school closures. See acceptable documents here;
  • Proof of household income meeting eligibility requirements.
  • Bank information including account and routing number if you choose to direct deposit. You can typically find this on a check
  •
